Your application answers will serve as information for us to evaluate a fit for a charitable fund at Tides and as the starting point of our future partnership if accepted.

Your application will not be officially submitted to Tides until you click the 'Submit' button. You will have an opportunity to review and correct required fields and to print your final responses before submission. Donor Advisor (primary contact)

A Donor Advisor has the authority to make recommendations regarding distributions from a fund, and to update, modify, or request other administrative changes to the fund. Unless otherwise established, each Donor Advisor to a fund has separate, full, and equal authority.

Fund Details and Investment Options

Fund Details

Please choose a name for your fund.  For example, "The Garcia Family Fund" or "Coral Reef Fund".


All grants from Tides are sent with award letters that generally include acknowledgement of the donor or Fund. Please select the default acknowledgement from the options below (acknowledgement can be updated on a per grant basis when making grant recommendations as well).


Investment Options
Tides provides donors with the option of opening an invested Donor Advised Fund, which permits donors to recommend that Fund assets be invested in Tides mission-aligned investment allocations. If you are interested in learning more about this option, please select “Yes”, and we will contact you to discuss our Investing for Impact resources and fees with you.

*A separate fee structure applies for invested Donor Advised Funds.

Investment allocations will be finalized upon DAF application approval.

Please note that the minimum initial total contribution for a Donor Advised Fund is $100,000.

Contribution transmittal instructions will be provided once the fund is approved.

Successor Advisors

A fund may maintain one generation of successor advisor(s), and/or may designate specific issue area(s) or organization(s) that would be supported by amounts remaining in the fund if the donor, donor advisor and/or any named successor(s) are unable or unwilling to make recommendations regarding remaining amounts, subject to review and approval of Foundation staff.

Review and Submission Instructions

You may also print and save a copy of your application from this page.  

At the bottom of the review page, click a final "Submit" button to submit your application to Tides.  An email confirmation that your application has been received will be sent to the email address provided on page 1 of this application.
